If you are a student looking to have a guest enter Curtis Hall, who is under the age of 18, you must contact Dean Davis for permission, at deidra.davis@mma.edu .

If you are submitting this information from the Athletics Department, Admissions Department, or Residential Life (Dean of Students, Director or Coordinator of Residential Life only) please download and type in the answers to the questions on the sheet designated for your department. Attach the document to the email and send the email to MMA Security Office .
